Enrollment Information & Forms

The following information is required to complete enrollment, registration, and class placement:

Student must have enrollment papers completed by a Parent/Legal Guardian to be eligible for enrollment. Completed enrollment forms must be presented to the Elementary office by the Parent/Legal Guardian. We cannot accept faxed/mailed paperwork. A student cannot be enrolled until ALL requirements have been met.
