Enrollment Information & Forms

The following information is required to complete enrollment, registration, and class placement:

• Completed Enrollment Forms

• Birth Certificate (for Kindergarten - child must turn 5 on or before July 31)

• Social Security Number

• Up-to-date Immunization Records

• Proof of Residency (Both the student and the Parent/Legal Guardian must reside in the Ash Grove School District)

          One of the following must include correct street address:

• • Rental Contract/Lease Agreement Receipt (must have renter's name included also)

  • Fully Executed Real Estate Contract

  • Current Paid Utility Bill

  • Personal Property or Real Estate Tax Receipt 

• Copy of valid Missouri Driver’s License/State I.D. of enrolling Parent or Legal Guardian

Student must have enrollment papers completed by a Parent/Legal Guardian to be eligible for enrollment. Completed enrollment forms must be presented to the Elementary office by the Parent/Legal Guardian. We cannot accept faxed/mailed paperwork. A student cannot be enrolled until ALL requirements have been met.
